To a male medical doctor who is married:

Dr. and Mrs. John McCoy

To a female medical doctor who is married:

Mr. and Dr. McCoy
123 Orchard Lane
Destin, Florida
90210

To a couple who are both medical doctors:

The Doctors McCoy
123 Orchard Lane
Destin, Florida
90210

To a member of the clergy:

The Reverend Michael Smith and Mrs. Smith OR
The Right Reverend Michael Smith and Mrs. Smith
123 Orchard Lane
Destin, Florida
90210

Additional Insights:
The title, Dr., should only be used in reference to a medical doctor. You should not indicate the title, Ph.D., to someone who has acquired a doctorate degree because they are not addressed as doctors socially, only in academia.
